hi i had worked for dhanalakshmi bank , its registered in thrissur kerala.. now i want to know about my establishment code

it should be there on your payslip.

I had worked my previous company (Apr '09 - Jun '11) in Madurai. I applied my PF amount two weeks back. Will i get my Pension Fund? (I came to know that PF calculated by Employee Share from my Salary + Employer Share from Company + Pension Fund)

Because, when i applied the PF amount, msg comes like that only (EE amt: Rs. XXXX; ER amt: Rs XXXX)... Thats why i am asking.... Could you clarify my doubt???

Yes, if you have filled form 10c and opted for withdrawal of EPS, you will get the EPS share too.

My service period is 4 years 10 months, whether i am eligible for eps if yes then how.

I have resigned & filled form 19 for EPF and my employer (Trust) sent me payment by chq. I want to know whether received amount include EPS share too (imp. to note that i have not filled form 10c for eps withdrawl).

Please guide me that if received does not include EPS then how can i get it now.

Yes, you are eligible for EPS. If you havn't fill 10C, you didnot get EPS amount. You can get it simply by submitting 10C and related documents.
